SMP Service Agreement

I, agree to the following terms of service below:

I understand that Scalp Micropigmentation is a multiple session procedure and I will not see the finished results until the entire procedure is complete.

The cost of the procedure stated above is based on my hairstyle today.

Price is subject to change if my hairstyle changes before the procedure is complete (e.g. shaving hair mid-procedure).

Complimentary touch-ups will be performed within 90 days of completion of the final procedure. No touch-ups will be done between full sessions prior to completion.

Touch-ups after 90 days will be charged by the hourly rate of $300/hr.

I have read and agree to follow the aftercare instructions provided and I fully understand that it is critical to follow the aftercare instructions for best results.

Payment for SMP services are non-refundable.
